1. The PhilCom Mas Yo! Dos Sales promotion is open to all PhilCom residential and commercial telephone subscribers, PhilCom Internet subscribers, PhilCom Public Calling Office Agents and PhilCom Payphone Agents (hereinafter collectively referred to as “PhilCom Customers”) and a mix of PhilCom Callkard Plus Users;

2. There shall be a total of twelve (12) draws within the promo period which are scheduled as follows:

3. A monthly draw is considered closed after the DTI representative has validated the result.

4. This promo covers the April 2004 Billing Period up to the March 2005 Billing Period.

Qualifications:

1. PhilCom Customers must bring with them their current PhilCom Statement of Account and pay their Total Outstanding Balance on or before due date to avail of the promo. Total Outstanding Balance refers to the amount reflected in the Statement of Account which is due for payment at the specified time therein.

2. New subscribers are entitled to participate in the raffle and are subject to the same rules and regulations.

3. Callkard Plus users must bring with them a minimum of Five Hundred Pesos (P500.00) worth of used-up Callkard Plus.

4. A PhilCom Customer and/or PhilCom Callkard User become an automatic qualifier of Mas Yo! Dos based on the following provisions:

• For a minimum of Five Hundred Pesos (P500.00) billing which is paid on or before due date provided that such payment is for the total outstanding balance of the applicable monthly bill;
• For a minimum of Five Hundred Pesos (P500.00) worth of used-up Callkard Plus, regardless of denomination;

5. After complying with the preceding paragraph, every Two Hundred Fifty Pesos (P250.00) is entitled to one (1) PhilCom Mas Yo! Dos Stub;

How to Play:

1. Mas Yo! Dos Stub is perforated into two parts. One part becomes a raffle entry for the minor prizes. This must be dropped at designated drop boxes of any PhilCom Business Centers. The other part which contains the three (3) distinct and pre-generated number combinations, ranging from zero (0) to thirty-five (35), becomes the entry for the major prize. This must be kept by subscriber for claiming purposes.

2. The Mas Yo! Dos Stubs are issued with a specified month in which it is eligible to be played. Mas Yo! Dos Stubs for a particular month cannot be played or used for any other month. After a draw has been officially closed, all Mas Yo! Dos Stubs for that month will automatically be invalidated.

Major Prize:

3. Under the supervision of a DTI representative, the major draws shall be conducted in any of the following:

• Cagayan de Oro City
• Butuan City
• Panabo City
• Surigao City

4. The winning number combination shall be determined by drawing three (3) distinct numbers, ranging from zero (0) to thirty-five (35), for each monthly draw of the promo. An entry stub valid for the major prize, shall be considered as a winning stub if the pre-generated number combination in it, corresponds, irregardless of sequence, to the drawn numbers.

5. The three (3) pre-generated numbers comprising the winning number combination is distinct from one another and, as such, no number shall be drawn twice or thrice in series or succession for a given draw.

6. The prize for the promo shall be at One Hundred Thousand Pesos (Php 100,000.00) per month. In case there are several winners, the prize shall be divided equally among them. If there is no winner for a particular month, the prize for that month shall be carried over to the succeeding draw/s.

7. The claimant winner/s must submit the portion of the stub containing the pre-generated winning combination within seven (7) days after each draw date. After the said seven (7) days, late claimant winners will not be entertained and declared automatically disqualified from that particular draw.

8. After complying with the preceding paragraph, PhilCom shall send notices to the validated claimant winner/s that his/their eligibility for the prize has been verified. The winner/s shall then redeem the prize within sixty (60) days after receipt of such notification; otherwise the prize shall be forfeited. All prizes may be claimed at the nearest PhilCom Business Center.

Minor Prize:

9. Under the supervision of a DTI representative, the raffle draw shall be conducted in the local exchanges.

10. All entries valid for the minor prizes shall be filled-up, completing the following basic information: subscriber name, account number, telephone number and signature.

11. All PhilCom Mas Yo! Dos entries qualified for the minor prizes shall be dropped in designated drop boxes at the following areas:

• Cagayan de Oro Exchange (Max Suniel St., Carmen, Cagayan de Oro City)
• Valencia Exchange (Mt. Kitanglad St. Purok 4, Valencia City, Bukidnon)
• Malaybalay Exchange (Lunsayan St., Brgy. Sumpong, Malaybalay City, Bukidnon)
• Gingoog Exchange (Mercado Avenue, Brgy 23, Gingoog City, Misamis Oriental)
• Butuan Exchange (JC Aquino Avenue, Brgy Imadejas Butuan City, Agusan del Norte)
• Cabadbaran Exchange (T. Curato St., Cabadbaran, Agusan del Sur)
• Surigao Exchange ( Narciso St., Brgy Washington, Surigao City, Surigao del Norte)
• Bayugan Exchange ( Yakal St., Brgy Taglatawan, Bayugan, Agusan del Sur)
• San Francisco Exchange (National Highway, Brgy 5, San Francisco, Agusan del Sur)
• Panabo Exchange ( Km. 31, National Highway, Gredu, Panabo, Davao del Norte)
• Sto. Tomas Exchange (National Highway, Sto. Tomas, Davao del Norte)
• Lupon Exchange (Aguinaldo Highway, Lupon, Davao Oriental)
• Mati Exchange ( Upper Salazar St., Mati, Davao Oriental)
• Bislig Exchange ( Abarca St., Mangagoy, Bislig City, Surigao del Sur)

12. All filled-up Mas Yo! Dos entries for minor prizes submitted to any Philcom Business Office on or before the specified monthly draw date, shall be raffled in order to declare a per LEC winner monthly.

13. Only permanent ink ballpoint pens or standard ballpens can be used in filling up the raffle stubs. Use of pencil, marker pens, whiteboard pens, pentel pens or other non-permanent, erasable, non-ballpoint type of pens shall render the entry invalid.

14. A monthly draw is considered closed after the DTI representative has validated the result.

Disqualifications:

1. Failure of the Major Prize claimant winner/s to submit the portion of the stub containing the winning pre-generated number combination means disqualifications in favor of PhilCom. The pot cash shall be divided only to claimant winner/s who were able to submit the winning stub.

2. Non-appearance of the claimant winner/s in publicity & promotional activities & non-participation in other activities related to such shall mean forfeiture, in favor of PhilCom. The pot cash shall be divided only to claimant/s who is willing to submit to promotional and advertising activities of PhilCom.

Minor Prize:
3. Any Mas Yo! Dos raffle stubs which were not dropped in PhilCom’s designated drop boxes will be automatically disqualified for that month’s draw and in the succeeding monthly draws.

4. PhilCom Mas Yo! Dos raffle Stubs with incomplete entries shall be disqualified from participating in the monthly draw, or if drawn, will be declared as invalid.

Major Prize:
5. Torn, tampered or defaced Mas Yo! Dos Stubs shall be declared invalid, especially if alterations and erasures are substantial.

Draw Details:

1. The monthly cut-off date for the submission of PhilCom Mas Yo! Stubs shall be on the second Friday of the month succeeding the month covered by the particular draw.

2. Failure of announced and confirmed winner(s) to redeem their winnings after 60-days from public announcement will mean forfeiture in favor of PhilCom. Forfeited prize to be added to next draw.

3. The winner shall claim the prize personally. In cases where he is unable to make such appearance, he may assign a representative to claim the prize provided that the latter brings with him a duly signed and notarized letter of authorization and two (2) valid identification cards of winner and the authorized representative.

4. PhilCom reserves the right to require the winner/s to appear in publicity & promotional activities & participate in other activities related to such.

5. PhilCom reserves the right to use the name and photo of the winner/s for advertising and promotional activities.

6. Prizes shall be awarded to the winner/s tax-free.

7. PhilCom employees and their relatives up to the second degree of consanguinity or affinity, and employees of PhilCom’s affiliate and subsidiary companies, are disqualified to join the promo.

8. PhilCom reserves the right to change, modify or amend any or all parts of these rules which it may deem inconsistent to this promotion.

9. PhilCom reserves the right to disqualify any entry found to be in violation of these official rules.
